    Description / Scope of Service
    The CV/CICU is a 24-bed Intensive Care inpatient unit that provides nursing care for adolescent, adult, and geriatric patients. The CV/CICU utilizes Evidence Based Practice to provide quality and effective care to our patients daily. Shared Governanceallows the staff to integratecore values and beliefs that professional practice embraces, and provides a framework that aligns nursing with UMC’s mission, vision, and strategic plan. Staff engagement and input are significant as we continue on our Pathway to Magnet journey.
     
    In the CV/CICU, the nursing care delivery model is Primary Care Nursing. The nurse-to-patient ratio is a goal of 2 patients per nurse and higher acuity patients require 1 nurse per patient. This can be adjusted by the charge nurse according to patient and staffing needs. This delivery care model dictates that the ideal candidate must be able to coordinate needed patient care with all disciplines.
     
    Nursing care is provided 24 hours a day, seven days each week.
    Nursing staff includes:
     
    • Nursing Department Director
    • Nursing Assistant Department Director
    • 2 Clinical Instructors
    • Registered Nurses
    • Nurse Technician
    • Nursing Assistants and/or Patient Care Assistants
    • Equipment Technician
    • Unit Assistant
     
     
     
    The following nursing diagnostic and therapeutic modalities are used to facilitate nursing care: nursing assessment and individualized care planning, IV therapy, cardiac/hemodynamic monitoring, post-operative care, safety management, skin care, respiratory care, psychosocial care, patient and family education, infection control, and medication administration. Nursing assessments are performed every four hours unless orderedotherwise. Vital signs are performed every hour unless otherwise ordered by Physician. Physician coverage consists of both private and TTUHSC faculty attending Physicians and resident Physicians.
    Floating to other units does not occur.
     
    A. Admission criteria is determined by, but not limited to, the following guidelines appropriate for admission to the CVICU/CICU. This list is not all inclusive.
     
    1. Invasive hemodynamic monitoring
    2. Urgent temporary pacemaker insertion
    3. Urgent Cardioversion
    4. Balloon pump (IABP) / Left ventricular assist device (LVAD)
    5. Acute intubation / Mechanical ventilation
    6. Nursing interventions at minimum of every 1 to 2 hours
    7. Post-surgery / procedure requiring monitoring at least every 1 to 2 hours
    8. Pre and Post ACBP
    9. Pre and Post Peripheral Vascular Surgery
    10. Pre and Post PTCA
    11. Pre and Post Cardiac Catheterization with intervention / Post Peripheral Intervention
    12. TAVR
    13. TEVR
    14. ECMO
    15. TANDEM Heart Assist device
    16. Hypothermia
    17. Watchman
     
    B. The following are examples of possible diagnoses appropriate for admission to the CV/CICU. This list is not all inclusive.
    1. Suspected or confirmed myocardial infarction
    2. Potential life-threatening dysrhythmias
    3. Pacemaker insertion or malfunction
    4. Unstable angina
    5. Elective Cardioversion
    6. Status post coronary angiogram or stent placement
    7. Patients who require continuous invasive arterial or pulmonary arterial pressure
        monitoring.
    8. Patient experiencing severe Congestive Heart Failure
    9. Patient experiencing cardiogenic shock to include Intra-Aortic Balloon Pump (IABP) therapy
    10. The patient population covers patients over the age of 15
     
    A multi-disciplinary approach to nursing care is utilized in the CV/CICU. Patient care providers will include Registered Nurses, Graduate Nurses, Nurse Technicians, Nurse Assistants/Patient Care Assistants, Respiratory Therapists, Pastoral Care, Occupational Therapists, Physical Therapists, Speech Therapists, Pharmacists, Social Services, Case Managers, Dietary, Patient Services, and Nursing Support Services.

    Individualized Unit Specific Orientation
    • Unit specific orientation is provided in addition to UMC’s Hospital and Nursing Orientation.
    • During clinical orientation you will be paired with an experienced nurse preceptor.  Our nursing preceptors are specially trained and work as a team to obtain the best experience for the new employee
    • Orientation length is individually based and will be determined by the Preceptor, Employee, and Clinical Instructor
    • The Graduate Nurse and new Registered Nurse can expect to attend critical care classes during the orientation period
    • Clinical orientation will begin on day shift to maximize your exposure to procedures and physician rounding 
    • You will also have many opportunities for professional growth and advancement
    • A CCRN Certification review CD is available (free of charge) after 1 year of employment
    • Committee involvement is encouraged in order to improve patient care and processes
    • Committees are available at unit level as well as hospital wide 
    • Other opportunities include: mentoring and the ability to self-schedule to meet individual needs
    • An Adult ICU Internship program is offered three times per year as needed.
    Qualifications
    • Graduation from an accredited School of Nursing
    • Valid Texas Nursing License or Compact Licensure  / Graduate Nurse Permit Required
    • Knows clinical and nursing standards and practices in accordance with the nursing scope and standards of practice as set forth by the American Nurses Association.
    • Provides professional nursing care to patients and provides supervision and guidance to the members of the nursing staff.
    • Age specific knowledge is required when needed for appropriate patient care.
    • Has working knowledge of organizational and nursing policies.
    • Requires adequate hand/eye coordination and fine motor skills.
    • Talking and hearing is essential for communication.
    • Visual acuity is required to work with patients for observation, assessment, and electronic charting.
    • American Heart Association CPR certification is required, free course provided.   
    • ACLS certification is required, free course provided.  
    • Customer service abilities including effective listening skills
    • Critical thinking skills, decisive judgment, and the ability to multi-task in a fast paced environment
    • Ability to perform work that requires walking, sitting, and standing most of the time while on duty. Occasional lifting or patients with assistance is required. Safe Patient Handling equipment accessible with training and use
