      Description / Scope of Service
      The Cardiac Intermediate Critical Care Unit (ICCU-5 East) is a 32-bed intermediate critical care inpatient unit that provides nursing care for adolescent, adult, and geriatric patients. The goal of this unit is to provide high quality care to patients no longer requiring critical care, but requiring a higher level of care than can be provided on the medical floors. The Cardiac ICCU serves as a step down unit for the Cardiac Intensive Care Unit. The nursing care delivery model is Modified Total Patient Delivery System. The nurse-to-patient ratio is a goal of 3-4 patients per 1 nurse. This can be adjusted by the charge nurse according to patient and staffing fluctuations and needs. This delivery care model dictates that the ideal candidate must be able to coordinate needed patient care with all disciplines. The ideal candidate must be able to make the education of patients and families a top priority. Floating to other units does not occur.
      Admission criteriato the Cardiac ICCU is determined by, but not limited to, the following guidelines:
      • Patients with low probability myocardial infarction (MI), rule out MI, and patients recovering from Acute MI
      • Patients with Unstable Angina
      • Patients post Primary Coronary Intervention (PCI) with sheath
      • Patients recovering from coronary angiogram that do not require thrombolytic administration
      • Patients awaiting arterial access discontinuation
      • Patients post pacemaker insertion or generator change
      • Patients requiring continuous telemetry or bedside monitoring
      • Patients with hemodynamically stable dysrhythmias that can be monitored by telemetry or bedside monitor
      • Post electrophysiology study patients including cardiac ablation
      • Patients with two or less of the following IV medications (requiring infrequent titration)
      • Patients with two or less of the following IV medications: Lidocaine/Procainamide, Amiodarone, Cardizem, Heparin, Dobutamine, Dopamine, Nitroglycerin, Integrellin, Cardene, Hypertonic Saline, Milrinone, Lasix/Bumex, Reopro, Cangrelor, andArgantroban
      • Patients with mild to moderate congestive heart failure (CHF)
      • Left Ventricular Assist Device (LVAD) patients, either pre-op, post-op or those requiring intermediate critical care
      • Stable Transcatheter Aortic Valve Replacement (TAVR) patients
      • Patients with hypertensive urgency without evidence of end-organ damage
      • Cardiovascular patients requiring respiratory support up to, and including, mechanical ventilation
      • Patients requiring nursing interventions every 2 hours for <8 hours (pulse oximetry, neurological checks, complex wound care, glucose monitoring); vital signs hourly for up to 4 hours; vitals every 15 minutes no greater than 2 hours
      A multi-disciplinary approach to nursing care is utilized on Cardiac ICCU. Patient care providers will include Registered Nurses, Licensed Vocational Nurses, Nurse Assistants/Patient Care Assistants, Respiratory Therapists, Occupational Therapists, Physical Therapists, Pharmacists, Social Services, Case Managers, and Nursing Support Services.

      Individualized Unit Specific Orientation
      • Unit specific orientation is provided in addition to UMC’s Hospital and Nursing Orientation.
      • During clinical orientation you will be paired with an experienced nurse preceptor.  Our nursing preceptors are specially trained and work as a team to obtain the best experience for the new employee
      • Orientation length is individually based and will be determined by the Preceptor, Employee and Clinical Instructor
      • The Registered Nurse, Licensed Vocational Nurse, Graduate Nurse, and the Graduate Vocational Nurse can expect rotations with the Telemetry Monitoring room, Cardiac Cath Lab, and with the Charge Nurse.  The new employee will participate in a combination of didactic (classroom) education and clinical orientation.
      • Clinical orientation will begin on day shift to maximize your exposure to procedures and physician rounding 
      • You will also have many opportunities for professional growth and advancement throughout and following your orientation period
      • PCCN Certification review class is available (free of charge) after 1 year of employment
      • Committee involvement is encouraged in order to improve patient care and processes
      • Committees are available at unit level as well as hospital wide 
      • Other opportunities include: mentoring and the ability to self-schedule to meet individual needs
      • Graduation from an accredited School of Nursing
      • Valid Texas Nursing License or Compact Licensure  / Graduate Nurse Permit Required
      • Knows clinical and nursing standards and practices in according with the nursing scope and standards of practice as set forth by the American Nurses Association.
      • Provides professional nursing care to patients and provides supervision and guidance to the members of the nursing staff.
      • Age specific knowledge is required when needed for appropriate patient care.
      • Has working knowledge of organizational and nursing policies.
      • Requires adequate hand/eye coordination and fine motor skills.
      • Talking and hearing is essential for communication.
      • Visual acuity is required to work with patients for observation, assessment, and electronic charting.
      • Current CPR certification from American Heart Association
      • ACLS may be required based upon area of practice.
      • Customer service abilities including effective listening skills
      • Critical thinking skills, decisive judgment and the ability to multi-task in a fast paced environment
      • Ability to perform work that requires walking, sitting and standing most of the time while on duty. Occasional lifting or patients with assistance is required.
