    Description / Scope of Service
    4 West provides care for any patient with a medical and/or surgical condition that requires continuous skilled nursing care to promote health and to strive for complete patient recovery or the return of the patient to the greatest degree of self-sufficiency.    The 4 West nursing unit is primarily a medical renal unit which also cares for other medical conditions.  The unit, which is jointly managed by nursing and medicine, consists of 30 private beds. In addition, the unit has a 4 bed, semi-private secured area used to provide care to inmates.   This area is secured by correctional officers that accompany staff during all care administered to the inmate.   The nursing care delivery model is Primary Care Nursing. The nurse-to-patient ratio is a goal of 4-5 patients per 1 nurse. This can be adjusted by the charge nurse according to patient and staffing needs. This delivery care model dictates that the ideal candidate must be able to coordinate needed patient care with all disciplines. The ideal candidate must be able to make the education of patients and families a top priority. Mandatory floating to other units does not occur.
    Admission criteria
    The admitting physician or his/her designee will determine the medical indications for admission to 4 West based on, but not limited to, the following:
    • Adult patients age 18 and older
    • Primary diagnosis of renal disorders such as: end-stage renal; acute renal failure; renal transplant rejection; renal transplant infections
    • Post-Op complications
    • Cardiac disorders such as congestive heart failure; pre and post cardiac heart surgery (ie: pacemaker placement); pre cardiac catheterization 
    • Diabetes
    • GI disorders
    • Indicated general surgical patients
    • Orthopedics (overflow)
    • Oncology (overflow)
    • General medicine patients such as pneumonia, COPD, hypertension
    A multi-disciplinary approach to nursing care is utilized on 4 West. Patient care providers will include Registered Nurses, Licensed Vocational Nurses, Nurse Assistants/Patient Care Assistants, Nurse Technicians, Respiratory Therapists, Occupational Therapists, Physical Therapists, Pharmacists, Social Services, Case Managers, and Nursing Support Services.
    The following nursing diagnostic and therapeutic modalities are used to facilitate nursing care: nursing assessment and individualized care planning, IV therapy,  intermittent cardiac monitoring,  peritoneal dialysis, parenteral and enteral nutrition, safety management, anti-coagulation therapy,  wound and skin care, respiratory care, general post-op surgical care;  moderate sedation for procedures, psychosocial care, patient and family education, infection control, and medication administration.  Special training programs include:   peritoneal dialysis and care of renal transplantation patients are provided to nurses who routinely work on the unit.  Chemotherapy treatments are only administered by nurses who have received appropriate training and education and have achieved and maintained a chemotherapy certification

    • Unit specific orientation is provided in addition to UMC’s Hospital and Nursing Orientation.
    • During clinical orientation you will be paired with an experienced nurse preceptor.  Our nursing preceptors are specially trained and work as a team to obtain the best experience for the new employee
    • Orientation length is individually based and will be determined by the Preceptor, Employee and Clinical Instructor
    • Clinical orientation will begin on day shift to maximize your exposure to procedures and physician rounding 
    • You will also have many opportunities for professional growth and advancement
    • PCCN Certification review materials and/or classes are available (free of charge) after 1 year of employment
    • Committee involvement is encouraged in order to improve patient care and processes Committees are available at unit level as well as hospital wide 
    • Other opportunities include: mentoring and the ability to self schedule to meet individual needs
    • Graduation from an accredited School of Nursing
    • Valid Texas Nursing License or Compact Licensure  / Graduate Nurse Permit Required
    • Knows clinical and nursing standards and practices in according with the nursing scope and standards of practice as set forth by the American Nurses Association.
    • Provides professional nursing care to patients and provides supervision and guidance to the members of the nursing staff.
    • Age specific knowledge is required when needed for appropriate patient care.
    • Has working knowledge of organizational and nursing policies.
    • Requires adequate hand/eye coordination and fine motor skills.
    • Talking and hearing is essential for communication.
    • Visual acuity is required to work with patients for observation, assessment, and electronic charting.
    • Current CPR certification from American Heart Association
    • ACLS may be required based upon area of practice.
    • Customer service abilities including effective listening skills
    • Critical thinking skills, decisive judgment and the ability to multi-task in a fast paced environment
    • Ability to perform work that requires walking, sitting and standing most of the time while on duty. Occasional lifting or patients with assistance is required.
