    Description / Scope of Service:
    The Post Anesthesia Care Unit has identified its scope of care to include all aspects of Post Anesthesia Care, specifically to assist the patient in returning to a hemodynamically stable condition after an anesthetic by providing safe, knowledgeable, individualized nursing care in the immediate post anesthesia phase.
     
    B. Patient/Customer Population Served
    Patient population includes infant, child, adolescent, adult and geriatric patients who have undergone a variety of general, orthopedic, gynecological, ophthalmological, genitourinary, neurological, cardiovascular, and otolaryngological procedures, performed on both an inpatient and outpatient basis. These procedures may be elective, emergent, or trauma in nature.
     
    C. Hours of Operation
    The Standards of PeriAnesthesia Nursing Practice provide the guidelines for postoperative care of patients in the Post Anesthesia Care Unit. These patients are cared for in the department on 24-hours a day, 7 days week basis by professional, licensed, registered nurses who are skilled in post-operative patient assessment and are A.C.L.S., P.A.L.S., and T.N.C.C. certified (optional). Ancillary and clerical support personnel are employed to augment patient care given by the professional staff, and to perform housekeeping and clerical duties.
     
    D. Employee Skill Mix
    Nursing staff includes the Director of Surgical/Anesthesia Services, Assistant Director/Clinical Coordinator, Clinical Instructor, Service Coordinator, and all staff are registered nurses. Patients are provided care through the primary nursing process and based on the Standards of PeriAnesthesia Nursing Practice. U.M.C. Environmental Services Department provides housekeeping.
     
    E. Staffing
    Post Anesthesia Care Unit patients are cared for only by professional, licensed registered nurses who are skilled in Phase I PeriAnesthesia care based on the American Society of PeriAnesthesia Nurses (ASPAN) recommended staffing ratio guidelines. Non-licensed and clerical personnel are employed to augment the patient care administered by these registered nurses to perform housekeeping, clerical duties and patient care as determined by the registered nurse.
     
    Admission criteriato the PACU is determined by, but not limited to, the following guidelines:
    Procedures are done in the Operating Room, Interventional Radiology requiring general anesthesia, MRI requiring general anesthesia, and Cath Labrequiring general anesthesia. Initiation of regional anesthesia, cast trims, emergency placement of chest tubes or tracheotomies, or pelvic examinations for bleeding may be performed in the Post Anesthesia Care Unit as indicated by individual patient requirements.
     
    The Main PACU consists of 11 (eleven) open ward patient care areas, 1 (one) negative pressure room, and 1 (one) closed isolation area. Day Surgery PACU consists of 9 (nine) and 1 (one) negative pressure isolation room.
     
    The Department of Anesthesiology provides medical direction and coverage with full time attending and resident physician staff. Medical coverage and direction is provided on a 24 hours a day, 7 days a week basis. Therapies include administration of oxygen, pulse oximetry, EKG monitoring, EtCO2 Capnography monitoring, administration of blood and blood products, IV drug therapy appropriate to the patient and diagnosis, as ordered, patient extubation, arterial/venous sheath monitoring & removal, and observation and evaluation of hemodynamic stability.
     
    A multi-disciplinary approach to nursing care is utilized on the PACU. Patient care providers will include Physicians, Registered Nurses, Nurse Assistants/Patient Care Assistants, Respiratory Therapists, Physical Therapists, Pharmacists, Social Services, Case Managers, and Nursing Support Services.

    Individualized Unit Specific Orientation
    • Unit specific orientation is provided in addition to UMC’s Hospital and Nursing Orientation.
    • During clinical orientation you will be paired with an experienced nurse preceptor.  Our nursing preceptors are specially trained and work as a team to obtain the best experience for the new employee
    • Orientation length is individually based and will be determined by the Preceptor, Employee and Clinical Instructor
    • Clinical orientation will begin on day shift to maximize your exposure to patient preparation in pre assessment/ pre op, post op Phase I & II and Surgeon/Anesthesia & OR staff hand off. You will also have many opportunities for professional growth and advancement.
    • Committee involvement is encouraged in order to improve patient care and processes Committees are available at unit level as well as hospital wide. 
    • Other opportunities include: mentoring and the ability for professional growth through national certification.
    Qualifications
    • Graduation from an accredited School of Nursing
    • Valid Texas Nursing License or Compact Licensure.
    • Knows clinical and nursing standards and practices in according with the nursing scope and standards of practice as set forth by the American Nurses Association and the American Society of PeriAnesthesia Nurses.
      • Provides professional nursing care to patients and provides supervision and guidance to the members of the nursing staff.
      • Age specific knowledge is required when needed for appropriate patient care.
      • Has working knowledge of organizational and nursing policies.
      • Requires adequate hand/eye coordination and fine motor skills.
      • Talking and hearing is essential for communication.
      • Visual acuity is required to work with patients for observation, assessment, and electronic charting.
      • Current CPR certification from American Heart Association.
      • ACLS/PALS may be required based upon area of practice.
      • Customer service abilities including effective listening skills.
      • Critical thinking skills, decisive judgment and the ability to multi-task in a fast paced environment.
      • Ability to perform work that requires walking, sitting and standing most of the time while on duty. Occasional lifting or patients with assistance is required.
