PART 1 – GENERAL JOB / PERFORMANCE INFORMATION
 
Education and Experience
Current licensure as a Registered Nurse in the State of Texas. BSN Master’s Degree (MSN, MHA, MPH etc..) required.  At least eight years clinical experience in acute hospital setting with experience in Quality or Risk Management preferred.  Previous leadership / supervisory experience as well as computer skills, research and basic statistical analysis is required.
 
Environmental Working Conditions
Subject to inside environment conditions.  Protection from weather conditions, not necessarily from temperature changes.  Hours may vary to meet specific deadlines.  Work is of light demand; frequent walking.  Frequent prolonged periods of sitting.  Lifting requirements of 20 pounds less than 10% of the time.  Frequent computer work with computer competency.

Overview / Summary
Reporting to the Vice President of Quality, primary oversight for facility-wide patient safety program. 
Directs process improvements supporting the reduction of healthcare errors that contribute to unintended patient outcomes / harm.  This includes (but not limited to) Safety Event Management, Risk and Safety Assessment, Continued Learning Environment Assessment (CLER) Survey, system-wide education as well as coordinates patient safety activities with other internal programs (ex: Lean and Six Sigma, internal audits) to promote organizational effectiveness.  Extensive training will be provided.
 
Required Licensures / Certifications / Registrations
Current Texas licensure from an accredited school of nursing.

PART II – JOB RESPONSIBILITIES
Technical
Every hospital employee is expected to display and acceptable level of technical competence in carrying out their assigned duties.  The specific job responsibilities for this position are listed below:
 
Job Specific
  1. Safety Event Management
    1. Responsible for planning, developing, implementing, and evaluating ways to reduce the number of health care errors that contribute to unintended adverse patient outcomes / harm.
      1. Educates clinicians and staff on the science of safety, utilizing Patient Safety Science Principles to build safer processes and systems (e.g. forcing functions, checklists, error causation thinking, human factors, situational awareness, applied informatics, culture).
      2. Focusing on reliability and applying the principles of reliable design to improve the consistent delivery of evidence-based care and reduction in preventable harm.
    2. Supporting and encouraging error reporting throughout the organization through a non-punitive just culture reporting system.
      1. Help to spread “lessons learned” from patient safety events, as well as successful initiatives, to other units and departments
      2. Educate and promote Near Miss / Good Catches as a way to identify future problems / areas at risk.
      3. Identifying opportunities to reduce variation in care delivery.
 
  1. Risk and Safety Assessment
    1. Conducts Healthcare Failure Mode Effects Analysis (HFMEA) as part of risk mitigation to determine where new and or current processes are susceptible not only to fail but potentially cause patient harm.
      1. Works with teams to ensure the actions plans of these HFMEAs are successfully implemented.
      2. Analyzes trends of performance metrics, identifies opportunities for improvement, develops action plans, measures success of these action plans and creates the records and follow-up metrics for each meeting and activity; reassesses approach to maximize success
      3. Provides education to UMC staff on how to conduct an HFMEA, including serving as an advisor.  .
    2. Conducts Root Cause Analysis (RCA) starting with an investigation of the adverse events through team facilitation to understand primary and or secondary causes of the event. Assists departments with an Apparent Cause Analysis (ACA) if a RCA is not required.
      1. Works with the RCA teams to ensure the corrective actions have been implemented and are sustainable.
      2. Analyzes trends of performance metrics, identifies opportunities for improvement, develops action plans, measures success of these action plans and creates the records and follow-up metrics for each meeting and activity; reassesses approach to maximize success
      3. Confers with appropriate management and medical staff regarding identified safety issues and the development and implementation of corrective measures.
  2. Reporting
    1. Reviews, analyses and aggregates patient safety data including trends and patterns that could identify process issues.
      1. Identify and classify the Organization’s risks (actual and potential), measure their frequency and report data to the appropriate administrator or committees.
      2. Monitors electronic surveillance of patients safety events
        1. Real time reporting of potential patient harm to the appropriate individual(s).
        2. Data aggregation to identify potential system issues.
      3. Provides support for:
        1.  Adverse event reporting and database maintenance for RL-Solutions, Patient Safety Organizations (PSO) as well as for mandatory state reporting.
      4. HFMEA and RCA/ACA, including trends, open corrective action plans beyond 3 months and other issues and or concerns, making recommendations for modifications based on identified and potential safety risks
 
  1. Medical Staff Engagement
    1. Working with the CMO and Chief of Staff as requested.
 
  1. Education
    1. Collaborates with UMC Marketing to increase awareness about patient safety and quality improvement initiatives.
    2. Participates in education of new employees and or others as requested.
    3. Utilizes best practice information from external organizations (e.g. ISMP, ECRI), to develop materials for inclusion in educational resources and programs.
  2. Other
    1. Assists in the CLER processes associated with TTUHSC as requested.
    2. Engages in Policy Management of assigned Administrative/Departmental Policies.
