    Description / Scope of Service
    The purpose of Case Management at University Medical Center (UMC) is to provide high quality patient care through appropriate resource allocation, and to assure effective and efficient utilization of hospital facilities and services.  It is the policy of UMC to base care on patient need and not on financial considerations.
    1. Scope of Service
    The Department of Case Management will perform the initial review to determine the appropriateness of the admission and the degree of case management the patient will require. The Department will assist in the coordination of the Plan of Care for the patient to include discharge planning needs and follow-up care.  The Case Manager will also be in contact with physicians, nurses, and support departments as needed to resolve any service issues or concerns.
    1. Patient/Customer Populations Served
    Case Management serves all patients, inpatient and observation, within UMC. Outpatient needs addressed on a case by case basis.
    1. Hours of Operations
    The Department of Case Management office hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. The Case Manager in the Emergency Center will work 9:00 am until 6:00 pm. The weekend Case Manager will work three 12 hour days on Friday, Saturday, Sunday and/or Monday. Times may vary. Transition coaches and 1115 Waiver staff hours will vary based on patients served.
    1. Employee Skill Mix
    All utilization review case managers are registered nurses (RN). Transition coaches and 1115 project managers are RNs. The director, assistant director, educator/team lead, and 1115 waiver/transitions coordinator are all RNs. Social workers are licensed and masters’ prepared. The community health workers are certified and/or will become certified within 12 months of employment. 
    1. Documentation
    Case Management will complete clinical reviews and fax out, as appropriate, via the utilization review software system. Discharge planning notes and other communications will be documented in Cerner under Clinical Notes, Ancillary Documentation, and Case Management.
    • UMC Hospital Orientation
    • Orientation as determined by the hiring department
    Individualized Unit Specific Orientation
    • Unit specific orientation is provided in addition to UMC’s Hospital Orientation.
    • During orientation you will be paired with an experienced nurse preceptor.  Our nursing preceptors are specially trained and work as a team to obtain the best experience for new employees.
    • Job specific duties are documented through the orientation period.
    • Orientation length is individually based and will be determined by Director/Educator.
    • In time, the CM will rotate through other units to be cross-trained for those units when coverage is needed.
    • Transitions/Waivers Case Managers can expect to spend time with the Educator/Team Leader and Waiver Coordinator in learning the basics of the Case Management department. The Waiver Coordinator will orient the Transition/Waiver Case Managers to individual project expectations. Other Transition/Waivers preceptors can be utilized to orient new Case Managers as designated by the coordinator.
    • All Case Managers will have opportunities for professional growth and advancement including nurse mentoring.
    • ACMA certification is available after 1 year of Case Management experience.
    • Committee involvement is encouraged in order to improve processes. Committees are available at a unit level as well as hospital wide.
    • Graduation from an accredited School of Nursing
    • Valid Texas Nursing License or Compact Licensure
    • MSN Required
    • 3 Years of Case management required. 
    • Knows clinical and nursing standards and practices in according with the nursing scope and standards of practice as set forth by the American Nurses Association
    • Has working knowledge of organizational and nursing policies
    • Requires adequate hand/eye coordination and fine motor skills
    • Talking and hearing is essential for communication
    • Must exhibit good judgment and problem solving
    • Letter writing skills required
    • Must be current in medical knowledge
    • Visual acuity is required to work with patients for observation, assessment, and electronic charting.
    • Current CPR certification from American Heart Association/ May or may not be required based upon level of patient contact
    • Customer service abilities including effective listening skills
    • Critical thinking skills, decisive judgment and the ability to multi-task in a fast paced environment
    • Ability to perform work that requires walking, sitting and standing most of the time while on duty. Occasional lifting with assistance is required

    • 3-5 years clinical nursing experience
